FORM 8500 – HOW TO GET YOUR CONFIRMATION NUMBER

Thank you for contacting Aeromedical for your Aviation Medical Examination (AME).

As of October 1, 2012 the FAA requires all 8500 form, flight physical applications to be completed online using MedXpress. Please use the link below to access this website.

This form can be completed 60 days prior to your flight physical examination.

Once the pilot has completed and submitted their part of the online medical application, a confirmation number will be generated from the FAA.

Aeromedical will require the pilot's confirmation number prior to the medical examination.

Under age 40, your First Class Medical is valid for 12 months before it will lapse into a second class.

Over age 40, your First Class Medical is valid for only 6 months before it will lapse into a second class

EKG's:

⇒The FAA requires that First Class Pilots must get a baseline EKG when are they 35 years old

⇒All Pilots holding a FCM after age 40 must get and EKG once a year
